This policy covers all Fossil Creek Farm Trust; it’s employees, contractors, sub-contractors, volunteers and clients.
Policy Details
We collect personal information from you, including information about your:
- Name
- Contact information
- Address and Contact information
- Health related information required for the Social Workers to operate with full health related information disclosed
- Your interactions with us
- Invoicing information
We collect your personal information in order to:
- Hold personal information relevant to providing our therapy services.
Besides our staff, we share this information with:
- Other health providers in order to assist with providing clients with the most beneficial mental health services available. This information will only be shared with your express permission.
We will keep your information safe by ensuring it is held in locked files and will only allow Fossil Creek Farm Trust Social Workers access to these files.
We will keep your information for five years or while your file is active at which point we securely destroy it
by disposing of it through a Confidential file destruction organisation.
You have the right to ask for a copy of any personal information we hold about you, and to ask for it to be corrected if you think it is wrong.
